外來人口換發新式統一證號提醒事宜

親愛的保戶您好：

    因應內政部新式外來人口統一證號專案(含居留證換發)自民國 110 年 1 月 2 日起實施，提醒您下列事項：

    若您已辦理變更新式證號且為本公司保戶並持居留證之外來人口(含外國自然人、大陸地區人民、香港澳門居民)，請您填寫「契約內容變更申請書」並附上更新後之居留證影本，以郵寄方式寄回本公司，以辦理保單資料更新。

    更多外來人口統一證號訊息，可至內政部移民署網頁查詢，或撥打移民署各服務站電話、1996內政服務熱線或外來人口在臺生活諮詢服務熱線(1990)。

Reminders for chang of Registration Information

 (New Uniform Identification Number) for Foreign Nationals

Dear Customers,

   In response to the Ministry of Interior’s “Replacement and Issuance of New UI No. for Foreign Nationals Policy” (also applicable to replacement and issuance of New Alien Resident Certificate (ARC)) effective on Jan 2 nd, 2021, please note that:

   If you have applied for your UI No. and you are a policyholder of Cardif Assurance Vie, Taiwan Branch and a foreign national (including foreign natural persons, people of the People’s Republic of China, and residents of Hong Kong and Macau) with an ARC, please mail the filled "Request for change form” with the photocopy of the latest ARC to Cardif TW to update your policy information .
